Do you want to live in a familiar environment and maintain your independence as you age? Our dedicated and passionate team of professionals provide a wide range of in-home care services to help you retain your independence. If you need to stay at home longer, we create an individual care plan together with you and your carer to support you to stay safely and happily in your family home. We2Care makes living at home easier; depending on how well you are managing on your own, we can offer a little, or a lot, of support around the house. Our staff can provide a range of services including companionship, personal care and skilled nursing.

We can help you with services such as:

  • helping with household tasks (light cleaning; laundry; ironing; changing bed linen; cleaning freezer, fridge and stove.
  • picking you up from hospital and staying with you until your carer takes over.
  • checking on you to make sure that you are comfortable and drinking enough water in hot conditions.
  • preparing meals for a day or week in your own home, and assisting with feeding.
  • helping you to shower and dress
  • shopping for you or with you
  • going with you on outings and day trips
  • assisting with personal hygiene and grooming (including toothbrushing, shaving, and hair care)
  • prompting you to take your medications
  • providing companionship and social support—we offer activities including games, walking, volunteering, reading, and long chats
  • assisting with mobilisation activities including turning, transferring, exercising and walking
  • providing continence care
  • referring to other service providers as needed
  • providing transport to appointments, shopping and social activities
  • providing respite to carers
  • maintaining the garden
  • performing welfare checks, either by phone or home visits
  • coordinating communication between service providers.
We have long experience in assisting individuals and families dealing with a range of conditions including Parkinson’s disease, dementia and Alzheimer’s. We know how challenging these conditions can be to the family, and we can help.

Community Nursing

We2Care nursing services are directed by experienced registered nurses and performed by enrolled endorsed nurses, enrolled nurses and assistant nurses in the comfort of your own home.

Registered nurses provide care plans that are based on a thorough examination that identifies your health conditions, the level of care required, and the appropriate medical interventions needed. Nursing is provided in consultation with your doctor and/or hospital discharge team. If there is an issue with your care and your GP is unavailable, We2Care nurses have access to our own in-house GP for clarification and guidance. At We2Care, we aim to provide the best possible care and management of your health issues.

Our team members receive regular supervision and continuous training to understand the diverse and changing care needs of our clients.

In-home Respite Care

From time to time, we all need a break to recharge our energy and increase our capacity to keep providing care. Respite care provides you with peace of mind, knowing that your family member is being well cared for by our qualified support worker. Our support worker will come to your home and take over your duties so that you can have a rest.

Respite care can be for:

  • a couple of hours
  • a day
  • an overnight sleepover
  • a weekend
  • a couple of weeks.

Social Work Services

Social workers at We2Care work towards improving the health and wellbeing outcomes of our elderly clients. We focus on preserving and enhancing functioning, and the quality of life for our clients.

What we offer

  • Assist you and your family to make significant life decisions
  • Provide mediation and conflict resolution around decision-making in the family when issues arise
  • Provide information about your rights and entitlements, available resources and complaints mechanisms
  • Advocate with you or on your behalf
  • Liaise with the hospital when you have been admitted to prevent unnecessary re-admissions
  • Carry out assessments and make referrals to service providers
  • Provide counselling to individuals, families and groups
  • Provide practical assistance, e.g. assistance with housing, appointments, moving into residential care
  • Coordinate formal and informal support
  • Support carers in their role
  • Help you to access community services, e.g. housing and government-funded services
  • Assist you with reading and completing paperwork
  • Write support letters to help you access services
  • Help you to navigate aged care systems
  • Provide key information about government benefits
  • Assist you with advanced care planning, including obtaining guardianship and/or administration orders if necessary

Post-hospital Care

Whether you are recovering from a serious illness or you require support and care after major surgery/treatment, or need support at home to maintain your independence, we can help. We2Care is committed to providing exceptional care that gives you and your loved ones peace of mind knowing that all your needs are being attended to. We listen to you and develop a package that suits your needs.

Our clinical team of registered nurses, enrolled nurses and assistant nurses ensure you receive the best care. This care is supported by We2Care’s GP in consultation with your family doctor. We2Care’s combined medical and non-medical model of service delivery helps make the transition from hospital facilities to home easier and safer.

We offer the following services:

  • Community nursing
